The transcript is the only source
The minutes are written after the recording ends, from the transcript and nothing else. No outside knowledge, no plausible guesses. If an owner or a deadline was never said out loud, that field stays empty — a guessed owner is a misassigned task, and that is worse than no task.
Every item carries its citation
Decisions, action items and open questions each record the transcript position they were derived from. A summary you cannot check against the transcript is one you have to take on trust, and in minutes trust is not evidence.
Some things stay unresolved on purpose
Anything that ended with "let me check" becomes an open question, not a decision. Refusing to tidy an unanswered thing into an answered one is what makes these minutes something you can act on.
